A caregiver moved a child by the wrist/arm and caused a nursemaid's elbow injury.

The operation has failed to provide proof of liability coverage by anniversary date as required.

During the walkthrough the operation was found to have reduced the number of caregivers for a classroom that included children under 18 months. This was corrected when the caregiver in charge placed another caregiver in the classroom.

The operation was not able to provide documentation of the current fire inspection received.

The operation failed to report an injury to a child that required medical treatment within 48 hours.

While attempting to redirect a child, a caregiver held a child's hand as the child resisted the grasp by dropping to the floor, resulting in injury to the child.

The toddler play structure had a screw sticking out of the play structure.

The operations first aid kit did not contain the tweezers.

During the inspection I observed the ceiling in a classroom to be in need of repair. There was also evidence of a leak in the ceiling. During the inpection of the playground. I observed sharp nails exposed on the playground fence.

Of the 10 training records evaluated, 7 did not receive annual training hours in administering medications, food allergies and handling, storing and disposing of hazardous materials.

Of the 10 personnel records evaluated, 5 staff did not receive pre-service training in SIDS, Shaken Baby Syndrome, Abusive Head Trauma and Understanding Early Childhood Brain Development.

A caregiver was allowed direct access to children in care without a request for background check being submitted.

The Employees who Left Employment in 2021 survey was not completed by deadline of 2/22/22.
