A child under 12 months old was observed sleeping in a restrictive device for five minutes. The child was finally moved to their crib when CCR staff prompted the caregiver to remove the child from the restrictive device. The child does not have a sleep exception form on file from a doctor allowing them to sleep in a restrictive device.

The bicycle track ride on the playground did not have the required six feet of use zone around it. It is less than six feet from a concrete footing that goes around the playground as well as the fence.

A child was observed sleeping in a restrictive device and also had a bottle laying on them that they had eaten out of.

Two of the four caregivers files reviewed did not have any of the annual training hours for the prior training year.

There were 5 children present in the infant room with one caregiver. The specified age group was 0-11 months therefore there could only be a maximum of 4 children with one caregiver. The classroom was over ratio by one child.

Documentation has not been received to show that the director meets director qualifications.

One of the seven staff files reviewed during the inspection did not have current CPR training. The CPR training on file expired in 10/2024.

An infant was observed on a Boppy pillow in their crib during the inspection. Staff interviews confirm that the child uses the boppy pillow to also sleep in an inclined position sometimes.

A child was observed in a room without a caregiver present and the child was visibly upset.

One of the infants at the operation did not have infant feeding instructions on file for review.

The activity plan in the toddler classroom does not have two opportunites for outside time. Additionally, the children in this classroom are only getting 25 minutes of the required 60 minutes outside.

During the inspection it was observed that the last inspection form dated 12/10/2021 was not posted. I observed Natalie print and post the form 2936 dated 7/27/2022 during this inspection.
