There were missing health statements from two children's records.

Information gathered during this investigation indicates that an employee made a child in care use mouthwash to ?wash out the bad words? after the child cussed at another child in care

There is evidence to indicate that the operation failed to notify CCR in the required 48-hour timeframe.

The water hose was left out on the ground in the playground.

When reviewing the personnel records, it was found that one record was missing the required notarized Affidavit for Applicants. The director stated that she feels that it was mistakenly put in another file as everything else was completed including the approved background check form, but indicated that if they could not find it, they would ask the caregiver to complete a new form and have it notarized.

Two classrooms, that sit next door to each other, were observed to be supervised by a paroling caregiver walking into each classroom during nap time. There was not a vantage point for all the children in both rooms to be supervised appropriately by a single caregiver.

During an investigation into other allegations, interviews provided information that a caregiver and a non-caregiver were both seen on two separate occasions to have used nicotine vaporizer pens inside the operation and while children were in care.
