During the course of the investigation, it was determined that a bathroom door was broken and fell on a child's hand which resulted in a child being injured.

In the Toddler class, there was a six seat toddler table. Zero of six children seated at the table were using the required safety straps. This was corrected at inspection when the caregiver strapped each child into their seat as required.

One employee record reviewed did not have the required 24 hours of annual training complete within their required annual training year.
