St. Paul Lutheran Early Childhood Center

A child was sleeping under a uncovered electrical outlet. This was corrected at the inspection when the caregiver placed a outlet cover on the exposed outlet.

There were four children files out of the six that were viewed, that did not have the date of the child's admission. Note: This was corrected at inspection with the director added in those dates to each of the children's files.

Conditions that were placed on a caregivers background check was not being followed. This was corrected when the caregiver was placed in a different room with another caregiver without conditions. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

A fire drill was not completed in January 2024. A fire drill was completed during the inspection and that children and staff were able to exit in 2 minutes and 41 seconds.

I observed bottles in a refrigerator that were not labled with the infants first name and last initial. The bottles were labled during the inspection

A bathroom that is counted in the operations capacity was being used as storage. I spoke to the director about keeping the restroom usable for the children as it is part of their capacity. The restroom was cleared out. A toddler playground was not being used due to maintanence that was needed on the playground. There was fencing around the AC that was cracked and the toys were knocked over, and the cover for the sanbox was in the sanbox bunched up which could cause a tripping hazard.

One medication was not labeled with the child's name and date brought to the operation. *This was corrected at inspection when the medication was properly labeled.

During the investigation inspection 2 of the diaper changing pads had dirt and debris under them. This was corrected at inspection when the changing pads were cleaned under.

During the investigation it was found that the caregivers were not following minimum standards. It is the directors responsibility to ensure caregivers are trained on minimum standards and check for understanding.

During the investigation it was found that a caregiver was leaving children unattended outside while going inside to help children go to the bathroom. It was also found that a child was wondering around the kitchen area without their caregiver. Another caregiver was present in the kitchen but did not acknowledge the child who did not have a caregiver. The child then went down the hall. It was a full minute before the caregiver came into the kitchen area with the class and noticed the child.

It was found during the investigation that a caregiver did not use good judgment when leaving children outside without a caregiver to take children to the restroom.
