There were three children in attendance without any enrollment forms or vaccination and Health Statements. The children have been in attendance for a week.

During a DFPS investigation inspection, the provider was unable to produce an employee's file who left employment on April 17, 2026.

Attendance records available for review were only available for four days in April and eight days in May. The remaining attendance record days for both months were not available for review.

It was determined during the investigation that children are made to cover their heads with their blankets while napping and are not able to remove the blankets.

The director completed 1 out of 30 annual training hours for the evaluation period reviewed.

One caregiver had completed 7 out of the 24 annual training hours for the evaluation period evaluated.

One of the two dogs present did not have current vaccination records showing they were up to date on vaccines available for review.

Caregivers were not able to demonstrate how to determine the ratio to follow with the combined ages of children in care.

Caregiver files, including training records, were not at the operation during the inspection. The director had taken them home.

In case of an emergency, there must be a qualified person in charge when the director is not present. This person must know they are in charge and know how to respond to situations that could place children at risk. The person in charge must also have access to essential records, which may need to be made accessible to licensing or parents when needed.

Children were observed on their mats with the lights off ready for nap time watching a movie.

The director completed 6 out of 30 annual training hours for the evaluation period of 7/2023-6/2024.

A caregiver without 2 yrs work experience and a hire date of 4/25/24 had not completed a preserve training.

Caregiver was not able to provide knowledge of the ages of the children in their care. Children in care ranged in age of 19 mos-4 yrs of age.

Caregivers were not able to provide knowledge of the ratio they were to follow with combined ages of children.

The copy of the operation's parent handbook provided to review did not contain a policy regarding parent rights.

A current staff posted did not contain the current employees.

The director's 1st Aid training expired 5/23.

The director's CPR certificate expired 5/23.

Upon arrival for inspection, there were 12 children and 1 caregiver present with a Specified Age Group of 2 years. The operation was over ratio by one child for approximately 30 minutes.

A child with a listed food allergy did not have a food allergy plan on file.

1 out of 2 caregiver files reviewed did not contain documentation of completed pre-service hours required due to caregiver not having 2 years of work experience.

1 out of 4 child files evaluated was missing the child's admission date. NOTE: Admission date was obtained and updated in the child's record during the inspection.

1 out of 4 child files evaluated was missing the complete address for the emergency contact person listed on the enrollment form. NOTE: The address was obtained and form updated during the inspection.

2 caregiver files were evaluated and did not contain employment start dates.

1 out of 2 caregiver files did not contain a notarized affidavit.

3 out of 4 child files evaluated did not contain emergency contact information.

1 out of 4 child files evaluated did not have a health statement on file.

1 out of 4 child files evaluated did not contain authorized pick up information.

2 out of 4 child immunization records wre evaluated and were missing updated vaccines.

One caregiver did not have documentation of receiving an orientation upon hire.

A child was observed sleeping on a rug in the library area during nap time.

A current activity plan was not available for review.

A caregiver had to leave the classroom to answer the front door leaving the children alone in the classroom.

Children came inside from outdoor play and sat down in circle time to read a book. Children did not wash their hands.

First aid kit did not contain cotton balls.

Background checks for the director and caregivers were not entered or processed prior to the inspection. NOTE: The provider has not been able to register for an online portal account.

Health statements were not available for review for the provider's 2 dogs on site.

The main restroom locks from the inside and there is not an available key to unlock the door should a child accidentally lock themselves inside.

There is a Little Tykes storage chest on the playground without a lid support mechanism preventing it from closing should a child climb inside.
