The Learning Experience

This rule is being evaluated as part of an investigation and found to be incompliance. Child was left unattended outside on the playground and was found by another caregiver in the building.

Caregiver did not follow operation's procedure when transition from playground to classroom. As a result, a child was left unattended on the playground for approximately 2 to 3 minutes.

A child was left alone in the outside playground for approximately 3-5 minutes.

Liability expired since 5/18/2024.

A child was left alone in the outside playground for approximately 2-3 minutes.

A caregiver is disciplined her own child inappropriately in front of other children.

2 children were not supervised properly as the caregiver was not aware of activities children were engaged in while in the restroom and therefore the caregiver was not able to intervene when children were engaged in inappropriate behaviors.

One of 10 children's file reviewed were lacking updated immunization records.

Caregiver did not share with the incoming caregiver that a child was in the restroom.

A child was not properly supervised when left unattended in a restroom for 10min.

Child's physician nor health care professional was contacted when a child in care at the operation turned blue and required CPR.

Parent was not notified immediately of an injury that required CPR and medical attention.

Caregiver did not use good judgment in allowing a child that required CPR to continue on playing, eating snack, and going outside without seeking medical attention. Caregiver did not use good judgment in not notifying the director and parent immediately after performing CPR on a child in care. Director did not use good judgment in not informing a parent immediately when she was informed that a caregiver performed CPR on a child 10 min after her initial call to the parent but instead waited over an hour to call the parent back.

Director did not run the operation in compliance when a parent was not notified immediately that CPR was performed on their child and required medical treatment.

When a child became ill at the operation and turned blue, EMS was not contacted.

Five of six infant feeding schedules were not signed and dated within the last 30 days.

Three of five infants did not have current feeding instructions on file for review.

One infant was observed in a bed with a loose fitting sheet. The sheet was changed during the inspection. A photo was taken of the loose sheet.
