Based on information gathered during the course of an investigation it was determined that a caregiver did not have visual awareness or knowledge of the ongoing activities while children were playing with scissors and therefore was not able to intervene when a child's hair was cut. Note: The incident occurred in August of 2023 and the operation has implemented new procedures regarding supervision since then.
