Children's Lighthouse at Heritage

At 7:23 am, during the walk-through of the operation, I observed 23 children in Room 5. There are 4 children who are 2 years old. The teacher confirmed the children's ages. The operation corrected this deficiency after I question the opening mix group ratio.

During the walkthrough, I observed that Room 7 children's bathroom (children aged from 2 to 4 years old) has cleaning supplies (Swiffer mop with cleaning solution) attached) in the children's bathroom.

A caregiver with provisional eligibility was observed to be caring for a group a children with no other qualified caregiver in the room with them. This was corrected during the inspection.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance with The Minimum Standards for Child Care Centers. A caregiver who is provisionally eligible with conditions did not follow one of the listed conditions when they were left alone in charge of the care of a group of children.

The operation did not report a situation that may have placed a child at risk when they did not notify Child Care Regulation that concerns were expressed about a staff member hitting a child on their hand.

A caregiver failed to provide supervision, resulting in the child being in the bathroom for over an hour.

A caregiver was observed to leave a class of children unsupervised for approximately 3-5 minutes.

During the monitoring visit, the operation had three medications that were expired for children enrolled in the operation.

All vans were missing complete first aid kits. This was corrected when the director added missing items to make each kit complete.

The caregiver did not exercise good judgment to assist the child off of the cots.

One of the staff files was missing a photo ID.

Three of the children's files that were reviewed did not have the emergency treatment completed and signed.

There were at least three incidents where a child was served food he was allergic to. The first incident a child was given a snack that had an allergy warning because it was made with milk. The second incident occurred when the same child was given a cookie that contained milk and eggs. The child is allergic to both ingredients. Another child was given a muffin made from milk. The child is allergic to milk.

A caregiver did not demonstrate competency when she ignored a food allergy notification that is posted in the room. She was aware that the child is only supposed to eat food brought from home. She was seen on video giving the child a cookie that was bought by the facility. As a result the child had an allergic reaction and was taken to urgent care. There were two other incidents where the allergy notification was ignored. The same child was given a snack food that contained milk. The sibling, also allergic to milk was given a product containing milk. These actions also resulted in an allergic reaction.

The facility failed to report a positve case of COVID at the facility.

The caregiver was heard, on various occasions, yelling at her children in the course of discipline. The caregiver was terminated as a result.

The caregiver was seen by multiple witness grabbing children by the arm. This was done in a harsh manner and in the course of discipline. The caregiver was terminated as a result.
