At the time of inspection, an inflatable swimming pool with slide is being used. The caregivers are not folloiwng the manufactuers instructions. There are more than six children using the inflatable pool at one time.

At the time of inspection the three year old classroom is out of ratio due to the specified age group being four years old. There are 22 children and the specified age range is four years old. They are out of ratio by four children.

At the time of inspection, the opration is using an inflatable swimming pool that holds over 18 inches of water. There is no certified lifeguard on duty.

Parents are not being routinely notified of minor incidents and injury occurrences at the operation.

At the time of inspection the infant room is out of ratio. There are eleven infants in the 0-11 age group and there are two caregivers.

Incident reports are not consistently being provided to parents.

There are exposed bolts on play structures that are not bolted down.

At the time of inspection there are seat belts that do not retract or fit properly on the van.

At the time of inspection two fo the vans in use are not equiped with working safety alarm systems.

Based on evidence from a DFPS investigation. A caregiver without an eligible background check was working at the operation.

One classroom had 3 more children than allowed for ratio with one caregiver. There were 18 children present in the room with a specified age of 3 years old, which allows only 15 children with one caregiver. The observation was made prior to nap time.

When reviewing video for 5/12/25 it was observed that a screen time activity exceeded one hour from between approximately 5:14pm to 6:30pm.

After a child threw their own shoe across the fence on the playground, the caregivers did not retrieve it, leaving the child without one of their shoes for the remainder of the day.

A child was diagnosed with nursemaid elbow

The allegation is related to a DFPS abuse/neglect investigation. Child was injured in 2023 that resulted in medical treatment.

This standard was found deficient as part of the DFPS investigation. The operation failed to notify parent of minor injuries.
