Two children were left unsupervised in a company van by a caregiver responsible for their safety during transportation.

During a DFPS investigation, it was determined that a child in care was in a restrictive device for more than 15 minutes. This was determined by observing video footage of the child in care.

During a DFPS investigation, it was determined that a caregiver who was counted in ratio, was using their cell phone for personal use. This was determined by observing video footage. This caregiver was providing care to infants and was observed to take a phone call.

The toddler classroom did not have an emergency preparedness plan posted. This was corrected at inspection when the director posted an emergency preparedness plan in the toddler classroom. This was corrected at inspection when the director posted an emergency preparedness plan in the toddler room.

In review of employee records, one employee did not have 16 of the 24 hours of required annual training available for review.

Two classrooms did not have an evacuation relocation diagram. This was corrected at inspection when the director posted an evacuation relocation diagram in the toddler classroom.

In the infant classroom, children's storage for belongings were not labeled. This was corrected at inspection when caregivers labeled storage area with children's names.

One caregiver working in the infant room did not have proof of training in sudden infant death syndrome available for review.

One infant in the infant room did not have an infant feeding schedule available for review for October 2025.

In vans used to transport children, 4 children did not have emergency medical transport and treatment authorization forms. Operation already has the children's admission form revised January 2025, sent to them previously by inspector from the TA Library.

In checking the two vans used to transport children, both vans were missing a thermometer. This was corrected at inspection when caregiver placed a thermometer in the first aid kits in both vans.

Caregivers did not have visual or auditory awareness of a child's on-going activity when a child walked out of the daycare unattended.

Operation did not have a fire drill documented for the month of July 2024 available for review. This was corrected at inspection when the person in charge conducted a fired drill, exiting the building in less than 3 months.
