The caregiver for the toddler classroom did not wash the children's hands prior to feeding the children lunch while Inspector was doing an observation for the child that had being injured before.

During a DFPS investigation, it was determined that one caregiver who had contact with children did not yet have an eligible status. This caregiver was present at the operation and had contact with children. The operation had not yet received notification from the CBCU that this caregiver was eligible, eligible with conditions, or provisionally eligible with conditions before allowing this caregiver to be present at the operation.

Four fire extinguishers had a receipt indicating they were purchased 12/19/2021. Fire extinguisher company serviced and tagged all extinguishers during the inspection.

Operation's last fire marshal inspection was conducted in 01/10/2022. A fire marshal inspection was due by 01/10/2023.

One out of four caregiver files was missing four hours of annual training for (01/13/2021 to 01/13/2022).

Fire marshal inpsection expired 11/05/2021.

Three infants not yet ready for table food did not have updated monthly feeding instructions for December 2021.

Infant caregivers confirmed they use the hand washing sink to wash or rinse infants' bottles.

Four vehicles had incomplete first aid kits. Some first aid kits were missing cotton balls, tweezers, thermometer, or scissors.

Fire extinguishers in three transportation vehicles were last purchased on 10/28/2020. Fire extinguisher in infant room and preschool classroom were purchased 10/28/2020 as well.

A child suffered a sprained ankle during a dancing activity. Staff was not able to explain to child's parent what had occurred. When parent picked up child, staff was not aware child had suffered a sprained ankle and did not know how to treat child's injury complaint.
