During a DFPS investigation, video footage was observed and it was determined that a caregiver counted in ratio pulled/grabbed a child in care.

A 6 week infant was observed swaddled in a bare crib while napping. This was corrected immediately by unswaddling the infant.

Drills were documented on a form but was missing the last three months of 2024. This was corrected at inspection by having the operation complete a fire drill and document it on the new form.

During an investigation it was found that a child in care sustained an injury that required emergency medical treatment due to not being adequately supervised. The caregiver responsible for this child had knowledge of a previous incident and did not provide the level of supervision necessary to ensure each child's safety and well-being.

It was determined a child was injured due the operation failing to properly secure and anchor a locker that fell on top of a child.

A teacher step out of the classroom to open the front door. The teacher left 10 school age children unattended. The person in charge was in ratio.

The hand washing sink in the infant room did not have running water. The infant room has not had running water 6 days.

During inspection I observed trash in the playground area. I observed a wooden barrier that has not been sanded or painted, which can cause injury to children in care. Pictures were taken at time of inspection.

During inspection while the caregiver was preparing lunch, the children were sitting at the table with no activity to keep them busy. Children were observed grabbing chairs, sitting for a long period and one child was observed putting his head down and falling asleep.

A feeding instruction form for an infant in care had not been updated since 5/1/22.

During investigation inspection it was observed a classroom of six children were left unsupervised when a caregiver left the room to take a plate to the kitchen. I also observed a three year old child running down the hall and entering the after school room by himself. The after school caregiver had to call the toddler/pre-k teacher to get the child from the room.

During the inspection an 11 month old infant was observed in a crib covered with a blanket.

During inspection I observed two children climbing on a table that could have caused injury and harm to a child in care. This was corrected when the children removed from the table and asked to get down.
