During the course of a DFPS investigation it was determined that a provider in care did not position themselves where could supervise all children adequately and could possibly prevent an injury to a child in care.

During the course of a DFPS investigation it was determined that the operation failed to report to Child Care Regulation an injury that required medical treatment for a child in care.

During the course of a DFPS investigation it was determined that the infant highchairs were placed underneath a shelf where a bottle warmer was placed; allowing for a child to have access to the bottle warmer cord who then pulled the cord which resulted in the child sustaining an injury.
