One vehicle used for transporting children did not have a fire extinguisher with a current service date, pin was missing, and needle was in the red. This was corrected at inspection when director replaced the fire extinguisher with one that has a current service date of 11/2023.

A vehicle (blue GMC bus)used for transportation, with Texas plate BML-5620, did not have a fire extinguisher. This was corrected at inspection when the director obtianed a fire extingsuisher serviced 11/2023 and placed it in the vehicle.

The restoom floor in the after-school classroom was not clean, with 3 empty toilet paper rolls and toilet paper was on the floor. Sink was dirty. There was no trash recepticle. This was corrected at inspection when caregiver cleaned up the restroom and added a trash recepticle.

In the pre-school class, two mats were not labeled during napt time. This was corrected at inspection when the caregiver printed labels and taped them to the mats.

The child safety alarm in transportation van with temporary plate 30F501A did not work when tested.

Two transportation vans had scissors missing from the first aid kits. This was corrected at inspection when the director added scissors to the first aid kits.

During a DFPS investigation, a caregiver was observed to be on their cell phone.

During a DFPS investigation, it was determined that a caregiver grabbed a child by the arm to pull the child up onto the caregiver's hip in order to carry them.

One of the transportation vans was missing a fire extinguisher.

Two employee files were missing their dates of employment. This was corrected at inspection when the dates were entered.
