Four out of six toilets in the operation were not working and were labeled out of service. The director reported the toilets stopped working on 12/29/25 and will call the plumber today, 1/06/25.

The lock down and sheltering drills were not conducted four times for the year 2025. Only one lockdown and sheltering drill was conducted for 2025.

Recite: There was one cigarette butt in a planter outside a classroom in the outdoor play area. There were two pieces of broken glass in the outdoor play area. Note: The items were removed.

Recite: The outdoor play area was observed with nails on the wooden panels. There was two small wooden pieces with exposed nails in a planter. There were three cigarette butts on the outdoor ground.

Recite: A bucket of water was observed in the kitchen area, where the door was open, creating accessibility for the children as they moved to their respective classrooms.

The outdoor play area was observed with dog feces, nails, and a battery on the ground. On an area of the fence there were exposed nails on the wood panels.

The transportation vehicle was observed with candy wrappers and coins (penny, quarter) on the floor which can be a choking hazard.

There was one caregiver caring for 8 children including 5 infants. The specified age for the group was 12-17 months which allows 5 infants to 1 caregiver. Note The ratios were left in compliance as an additional caregiver was put in the classroom.

During naptime, a classroom was arranged in a manner that did not provide adequate space between the mats utilized by the children. This resulted in one child inadvertently placing their leg over another due to the constrained spacing.

A bucket of water was observed in the kitchen area, where the door was open, creating accessibility for the children as they moved to their respective classrooms.

Outdoor area had a pile of trash, plastic knife, and empty can easily accesible to the children.

One classroom did not have activity plan available. Correted: This was corrected at inspection as director posted current activity plan.

Caregivers did not sanitize diaper changing area or change the non-absorbent paper liner between diaper changes.

A crib labeled with another infant's name was in use by an infant. NOTE: This was corrected during inspection. Infant was moved to their assigned crib.

Children's products certification was not completed timely. Last completed certification was completed on 7/7/2021.

The following trainings were not available for review for Director of operation: (1) Emergency preparedness. (2) Preventing and controlling the spread of communicable diseases, including immunizations. (3) Administering medication, if applicable, including compliance with 746.3803. (4) Preventing and responding to emergencies due to food or an allergic reaction. (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic. (6) Handling, storing, and disposing of hazardous materials including compliance with 746.3425.

The following trainings were not available for review for two caregivers: (1) Emergency preparedness. (2) Preventing and controlling the spread of communicable diseases, including immunizations. (3) Administering medication, if applicable, including compliance with 746.3803. (4) Preventing and responding to emergencies due to food or an allergic reaction. (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic. (6) Handling, storing, and disposing of hazardous materials including compliance with 746.3425.

CPR and 1st aid trainings were not available for review for one caregiver.

Caregiver did not wash hands after diapering a child.

Two caregivers were missing training hours documentation. One of the caregivers was missing additional required records.

A 16 month old infant was in the same group of children as 3 year old children. Operation was deficient in the 18-month rule standard.

Emergency medical transport and treatment authorization forms for each child being transported was not present in the transportation vehicle used on this day.

The use zone of a rotating device was deficient as observed during inspection. Another play equipment and a table were less than 6 feet away from the rotating device. NOTE: This was corrected during inspection. The table and play equipment was moved to comply with use zone.

There were 21 children present. 17 children were signed in to care.
