Based off the Department of Family Protective Services investigation, a child was left unsupervised in a transportation van.

Based on the DFPS investigation, the director failed to call 911 after being notified by a staff member that a child was left in a transportation vehicle for approximately 3 hours. The director also signed and presented the parent with an incident report which documented inaccurate information.

This standard was found deficient as part of a DFPS Investigation.

Based on the DFPS investigation, the operation did not immediately notify a parent after their child was involved in a serious situation. At 12 PM the operation became aware a child had been left unsupervised in a transportation van, however the parent was not notified of the situation until pick up time, at approximately 5 PM.

Based on the DFPS investigation, the caregiver did not account for all the children prior to exiting the transportation vehicle, resulting in a child being left alone in the transportation van for approximately three hours.

Based off the Department of Family Protective Services investigation, pertinent information was being withheld from the Department of Family Protective Services investigator. Video surveillance was also being withheld from the Department of Family Protective Services investigator.

Based on the DFPS investigation, the permit holder and the director both showed a lack of judgment when they told the parent their child was left in a transportation vehicle for about 15-20 minutes with the air conditioning running, however it was later discovered the child was actually left in the transportation van for approximately 3 hours with no air conditioning on in the vehicle.

Based on the DFPS investigation, the permit holder did not call 911 and directed her staff not to call 911 after a staff member notified the permit holder that a child had been left in a transportation vehicle without air conditioning for approximately 3 hours.

A caregiver stated to placing a child in timeout for minutes older than child's age.

It was observed that a caregiver seats a child in restrictive device as a form of punishment.

A child was given a snack on a table.

2 of 7 caregivers were missing two required trainings.

The center did not have in a prominent place the local child care regulation office address or number.

The center did not have in operational policy on how parents can view minimum standards online.

Based off the DFPS investigation, A child was left unsupervised while using the restroom outside the classroom, without a caregiver verbally checking in or asking another floater teacher to supervise. This resulted in the child leaving the facility through the front door.

Based off the DFPS investigation, the caregiver was seen on video cleaning a table with her back to children as the child left the room.

The center did not have special care needs information on 5 of 5 children's files reviewed.

The operation did not have a suspension and expulsion policy, promotion of indoor and outdoor activity and services to include children with special care needs in operational policy.

After changing the diaper of one child the caregiver did not wash hands before reaching to pick up another child. This was corrected at inspection when licensing asked the caregiver to wash hands before picking up another child.

A classroom with children under 5 years old had 5 electrical outlets exposed with no covers. This was corrected at inspection when caregiver placed covers on all the outlets missing covers.

The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.

The infant classroom had 5 children between the ages of 3 months - 9 months making the classroom over ratio by one child.

Based on the DFPS investigation, school-age children were allowed to go back and forth to the bathroom without being supervised. Caregivers also left children unsupervised in the classroom.

There was no one left in charge of the operation while a child locked themselves in a restroom.

The director was lacking 8 hours out of 30 annual training hours for the 2021-2022 training year.

The loose fill under surrounding the playhouse with the climbing wall and stairs does not have 6 inches.

Three drivers did not have a valid transportation training. The last transportation training was taken in December of 2021 and it expired in December of 2022.

Two classrooms were observed to be out of ratio. An infant classroom had 6 children with one caregiver and the specified age group was 12-17 months. The classroom was one child over ratio. The second classroom had 15 two-year old children to one caregiver. They were 4 children over ratio.

Three out of eight children's immunization records reviewed. One child did not have an immunization record on file. Two other children did not have an up to date immunization record on file.

Two out of four staff trainings that were reviewed did not have the 24 hours of annual training. One caregiver was lacking 4 hour of annual training for the 2021-2022 training year and was lacking 23 hours of annual training for the 2022-2023 training year. The other caregiver did not have any trainings for the 2021-2022 training year.

The infant classroom had 8 children with one caregiver and a specified age group of 12-17 months they were over ratio by 3 children.

A small playhouse was observed to be broken and had sharp edges. An outside white board was observed to be broken, had sharp edges and was rusted. Corrected at inspection. The playhouse and the white board were removed from the playground during the visit.

The operational policy lacks the required outdoor policy.

Four out of ten children's records reviewed were lacking health statements on file.

The operational policy lacked the infant safe sleep policy.

The fire inspection posted at the center was last conducted on 04/12/2021 and is expired as of 04/12/2022.

The infants were not taken outside to play, and outdoor play was not listed on the activity schedule.

The menu posted said they were serving bean and cheese tacos, but they served cereal, milk and apples. Corrected at inspection: The menu was corrected during the inspection.
