Caregivers in two classroom did not know the number of children in care during inspection.

The operation liability insurance on file expired in January of this year and has not been updated within the required annual period.

A caregiver did not use good judgment during naptime as the caregiver pushed the child toward the nap cot which caused an injury to the child.

Based on video footage, it was determined that caregivers have not been strapping infants into the chairs when being used.

It was determined that the center is not properly cutting up hotdogs when served to the children.

During naptime, the 12 to 17month old classroom had 1 caregiver and 9 children.

The operation does not have a medication on file that is listed on the food allergy action plan.

Staff were not properly supervising children which resulted in a child being able to play with a locker, which the locker then fell on the child.

Two staff were missing the remaining 16 hours of pre-service training.

Five infant feeding instructions have not been updated.

Four volunteers were present at the operation and were in classrooms with children without a background check.

An infant was observed in the pack-n-play asleep with an inappropriate sleep sack on.

The following was missing during the follow up inspection; -Two children's files did not have the Emergency Care Authorization completed. -Two children's files did not have the special care statement. -One child file was missing a health care statement.

One staff file does not have documentation showing how the employee meets the education qualifications.

Ten children's files were observed and missing the following; -Ten children's files did not have the Emergency Care Authorization completed. -Ten children's files did not have the special care statement. -One childs file was missing a health care statement.

During the inspection, it was determined that children were placed in the seats at the table and not strapped in. There are no safety straps in the seats.

During opening time, a movie was being played on the TV, and an 18month old was in the classroom.

Breakfast time was observed and children did not wash their hands before eating.

The operation has loose fill that measures 0 inches at fall zones within the use zone of play equipment.

On the playground, there was a broken piece of the border that contains the loose-fill.
