The operation's policies does not include a promotion for indoor and outdoor activities section.

Based on information gathered through inspection, most of the operations personnel records are missing information and cannot be evaluated.

The operations director and two assistant directors have not been maintaining attendance records.

Based on information gathered through inspection and observations made, a 4 month old infant that is not able to roll over was placed on their stoomach to sleep.

A caregiver did not use good judgement during diaper changing of a 2 year old, when she fixed the broken tab of the diaper with a masking tape and used it instead of using a different new diaper that was readily available for her.
