A child present at the center did not have completed admission form. This was corrected during the inspection when the child's guardian completed the admission form for the child.

There were 3 children under two years old observed actively using screen time.

One of the outdoor playground's loose fill measured at less than 4 inches. The second outdoor playground's loose-fill measured at less than 1 inch.

The operation was unable to provide documentation of the Children's Product Certification for the current year. The form posted is dated 4.19.2023

An electrical outlet did not have a child proof cover in an observed classroom. This was corrected when the operation placed an outlet cover on the outlet. One of the observed classrooms had a cabinet with cleaning supplies inside did not have a child proof lock. This was corrected when the operation placed a child proof lock on the cabinet. One of the observed classrooms had a can of air freshener spray within reach of children. This was corrected when the caregiver placed the air freshener out of reach of the children. Two large holes that could cause a tripping were observed in the outdoor playground area. There were two ant hills in the outdoor play area. There is a broken ladder and table in the outdoor play area

Two stationary equipments, a rocking equipment, and two rotating equipments, have overlapping use zones on the pre-k playground The toddler playground has two overlapping stationary equipment.

The operation was unable to provide the playground checklist. The last daily/playground checklist was completed in 12/2025.

The observed annual trainings for the 2024-2025 training year did not have the required training for caring for children under 24 months.

A 7-year-old child was allowed to ride in the front seat of the operation's van while being transported from the operation to the child's school.

There was not a caregiver in close proximity to a child with special needs and the child was able to exit the center using the back door.

A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

The cribs sheets were loose fitting.

One caregiver did not have the required diploma or equivalent.

The playground area around the slide zones were in need of loose fill.

During the inspection it was found that the caregivers were using the Brightwell App and it was not showing the children's general mood, their summary of daily activites and the proper amount of diaper changes. The Director is going to correct this by starting to use a paper form of the infant daily report and sending copies of the reports to Licensing by the compliance date.

Five out of nine children's records evaluated was missing the health statement. The Director is going to corrct this by getting the health statements.

There were six infant feeding instructions that had not bee updated. The Director is going to correct this by getting the feeding instructions updated.
