One caregivers file is missing documentation show that they received and reviewed the operational policies and personnel policies. Note: The caregiver completed the personnel form 2947.

While outside on the playground the school age caregiver could not adequatly supervise the children as half the group were not within eye sight on the playground as they were on the other side of the building.

One caregiver is missing the topics under this standard.

One caregiver does not have documentation showing that the meet the caregiver requirements. Note: The caregiver completed the application information showing that they meet the requirements.

Two children's files have a parent listed as an emergency contact instead of an alternate adult.

In the school age room: The boarder is coming off of the wall near where the cots are kept. The floor pannels are shifting causing large gaps. Paint is peeling along the back wall near the vending machines.

One caregiver has not signed off stating they have completed pre-service and orientation trainings. Note: The caregiver signed the personnel form 2946.

One child's immunization record does not have the doctor's signature and information on the record.

One caregiver does not have the pre-employment affidavit in their file.

Two caregivers are missing the signed and notarized affidavit in their files.

The school age children do not have individual cubbies or space for their personal belongings.

Two children did not have completed shot records in their files. Note: Parents to both children sent in current copies of the shot records during my inspection making this corrected at the inspection.

The loose fill under and around the climbing structure was not 6 inches per required. Note: The director and caregiver blocked off the area of the climbing structure and loose fill so no children can play in the area until repairs can be made. This brought it into compliance.

The caregivers and director do not have current CPR. Their CPR expired in June of 2025. Note: The director informed me it was scheduled for 6/21/25; however, due to the AC breaking and repairs needed the class was set to be rescheduled.

The climbing structure was observed to have several spots where the metal was rusting and the protective covering was peeling away from the stairs and platform. The boarder around the climbing structure is broken in several places causing sharp edges. Note: The director and caregiver blocked the entire climbing structure and loose fill area off until they can get it repaired. This brought it into compliance during my inspection.

Follow up 11/14/24: There is still roach activity in the kitchen. Note: Pest control has been twice since the last inspection.

A free standing arch climber that does not have loose fill under or around the use zones.

Vents in the toddler room were visibly dusty. A fan in the infant room was dusty. Note: The vents were cleaned and the fan removed from the room during my inspection.

Sign in and out logs were not readily available for a few of the caregivers and the director during my inspection.

Roach activity and live roaches were observed in the kitchen area. Note: Pest control does come out regularly. Pest control was scheduled for Friday with a request for a different formula.

At least three of the smoke detectors were beeping indicating low batteries. Note: The batteries were replaced during my inspection.

The emergency drill log could not be found during the inspection.

One child's file out of six reviewed does not have current immunization records.

The air vents in the school age room were visibly dusty. Note: The vents were dusted during my inspection. Pictures were taken of the dirty vents and clean vents.
