A mobile infant was observed on the floor by the door of the infant classroom with a piece of lettering that was pulled off of the door in the infants mouth that could cause a choking hazard.

2 cribs in the infant room were observed to have sheets that were too big to fit the mattresses causing an entanglement hazard.

An infant was observed asleep in the crib with a bottle.

Cribs in the infant room weren't labeled with each infant's name or numbered to identify each infant's assiigned crib.

The field trip group was out of ratio on a field trip that required the ratio to be 1 caregiver for 12 children with the youngest child being 6 years of age and older. The caregiver was supervising 14 children.

The operation's vehicles don't have current registration.

One out of 10 children's records I evaluated did not have health statement.
