This standard was tasked as part of a DFPS investigation and found to be deficient. Caregivers inappropriately grabbed a child in care.

This standard was pulled a part of an investigation and found to be deficient. It was determined that caregivers did not provide care consistent with the child's potty-training habits leading to the child being sent home three times soiled.

The operation allowed a caregiver to care for children before receiving a background check determination of Eligible, Provisional Eligibility with Conditions, or Eligible with Conditions. The caregiver left during the inspection.

This standard was evaluated in regards to a investigation and was found deficient but it was corrected at inspection. The policy did not acknowledge a open door policy but it was rewritten in their policy and it was sent to the parents via email.

Based on observation and interviews, the operation did not have an open-door policy. Parents were not allowed in the operation during morning drop-off.

The director did not ensure qualified substitutes who had proper eligibility, training, and documentation were called to care for children.

Eight children records were reviewed and one lacked a health statement immunization records, emergency care authorization, and heath care information. Another lacked immunization information. And lastly, a record lacked a health statement, immunization information, and emergency care authorization.
