The provider did not obtain the required health topics during the previous annual training year.

The operation did not notify within two days of being informed of the diagnoses to Child Care Regulation of a child that attends care at the operation of Salmonellosis.

The fire drill practiced and monthly checks of fire extinguisher, smoke detector, and carbon monoxide for the month of February 2022 was not completed. *This was corrected at inspection when the provider documented the fire drill for February and the testings/checkings of the fire extinguisher, smoke detector, and carbon monoxide.

At the time of inspection there were seven non school-age children present with one of those children being an infant. The operation is over ratio as there can only be six children present.

The fence had a hole that had been dug up from the ground and an adult hand could easily fit through the hole and it posed a hazard and risk for a child as the play area was not properly enclosed.

The condition regarding personnel records: For employees and household-members, prior to the start of the corrective action probation the provider must check the personnel records and document it on the Personnel Record Evaluation checklist provided by Child Care Regulation or a form that contains the same information. This must be completed within 14 days of the start of the corrective action probation start date was not completed. There are no new house hold members or employees and therefore, this was not completed within 14 days of the corrective action starting for the three house-hold members that are over 14 years of age. **The operation corrected this at inspection when she reviewed the files for the three house hold members and completed the checklist. This is in compliance.
