The director's training was incomplete. Documentation provided only showed 24 out of the 30 clock hours was completed.

2 caregivers did not have current training certificates in CPR and First Aid training. The certificates provided were both expired.

The operation had only completed 3 out of the 4 required sheltering and lock down drills within a calendar year.

Caregivers did not fasten the safety straps when placing children in their chairs. 6 of the chairs in the One's classroom were missing the safety straps.

Multiple infants feeding instructions have not been signed and updated within the last 30 days.

The annual fire inspection has not been conducted since May 2024.

The diaper changing mat was torn in the infant room and cannot be cleaned properly. Note- The provider replaced the changing mat during the inspection.

A child with a known food allergy did not have an allergy preparedness plan available for review.

During the inspection there was not adequedate lighting in the toddler classroom. Note: Caregivers adjusted the light.

During the inspection the operation did not have updated liability insurance. Liability insurance reviewed was from 2022.

During the inspection, I observed storage areas containing storage bins, an unused door with hinges, boxes, cleaning supplies, and a gate all accessible to children in care.

I observed hazards in the kitchen area, opened storage rooms, and in the restrooms.

The operation does not have a current annual fire inspection since 06/20/2019.

The operation has not update their children product certification since 2020.

The operation has not done their annual fire drill for the month of October.
