The operation was over in ratio by 3 children during the inspection. The ratio was 38 children to 2 caregivers.

Caregiver did not know the correct number of children they were responsible for during the inspection.

Based on the information obtained, caregivers did not have an accurate count of children when conducting a name to face check. There were 17 children present in the classroom, and they only counted 16.

Based on the information obtained, caregivers left a child alone in the classroom unsupervised.

While reviewing personnel records 8 out of 10 files needed 8 hours of training in order to meet the 24 required hours.

While conducting a Health and saftey audit there was no annual fire inspection on file. The last fire inspection was conducted in April 2025

While reviewing personnel records there was 1 out of 10 files that did not have the skills based training portion completed,

During the walkthrough an infant was sleeping in the crib with a loose bed sheet.

During the walkthrough multiple sleeping mats were torn and can not be properly cleaned.

While reviewing the personnel records 4 out of the 10 files reviewed did not have a pre-employment affidavit.

Unitary surfacing material on the playground is torn.

Personnel files evaluated were missing the following information: - Completed Employment Affidavit Form - Proof of Education - Completed Orientation

4 children who were present were not signed into the operations tracking system during the inspection. Note: The provider signed each child in during the inspection.

A caregiver with a provisional background check was observed in classroom alone with children.

During the inspection a child in care medication was out of date. Note: Parents were contacted regarding the medication. The medication was discarded.

Based on the information gathered during the investigation, the operation failed at changing a child`s diaper in care throughout the day.

Based on the information gathered during the investigation, the operation was deficient.

The operation did not have the most updated gas inspection documentation to review during business hours.

I observed the emergency practice log for the Carbon Monoxide Alarm Detector, Smoke Detector Test and Fire Extinguisher Check not up to date.

A caregiver was in direct care of infants without an active background check.

The operation did not have an up to date fire inspection report available to review during business hours.

During the inspection the operation did not have the playground maintenance checklist up to date as required per minimum standard guidelines.

The operations director and all staff did not have the up to date transportation training on file during business hours.

Based on the information gathered during this investigation, the caregiver did not have an up to date First- Aid/CPR on file and was the only teacher in the classroom.

Based on the information obtained during this investigation the caregivers at the operation did not properly supervise the children in care.

The annual health inspection expired 8/6/2020 and requires renewal.
