Based on the child supervision record (Attendance Log) there were sixteen children ages 2 years old being supervised by one caregiver in Classroom Two's B on 07/29/2022 from 8:43AM-9:10AM until another caregiver came in at 9:12AM. The maximum number of children in this age group that one caregiver may supervise is eleven.

The operational staff did not maintain supervision when a child was injured in the infant playground.

Two classroom caregivers were unaware of how many children they had in their care during the inspection.

The caregiver on the playground had 19 children in her care during the inspection. The ratio is 18 to 1 the class was out of ratio.

A staff member did not demonstrate good judgment when physically redirecting and relocating a child to a classroom. The child resisted the redirection, yet the caregiver continued to physically redirect the child. Another caregiver intervened to better assist the child's transition to the desired classroom area.
