There was a caregiver supervising children without an active background check. The caregiver was removed from the classroom and sent home.

The operation had an expired Epi Pen on file for a child with a food allergy that expired on 12/31/2025.

The caregiver acknowledged responsibility for improperly lifting the child. She affirmed her awareness that the correct method to lift a child is from under the arms.

The director stated the video was observed and the caregiver was observed hitting the child on the bottom.

During the investigation, information was obtained the child was left unsupervised in the hallway.

A caregiver held a child?s wrist as a form of redirection, so the child wouldn?t hit staff and other children.

Based on the DFPS Investigation, a child in care was physically abused.

The director placed the child back in the classroom with the caregiver who displayed inappropriate discipline.

The caregiver did admit to using her cellphone while supervising children

A parent stated they heard a caregiver using profanity toward the children.

The operation had 2 expired epi pens of children in care. The director placed the epi pens away and contacted the parents to come and pick them up.

There were two classrooms that had cleaning spray on the counter which is accessible to the children. Director did removed the cleaning sprays and placed them in a cabinet.

All playground areas has not been maintained with loose filled.

Caregiver did not know the name of the children in her classroom. Caregiver did not have a list of the children in her classroom.

2 out of 3 children who receives an epi pen does not have a food allergy emergency plan.

The operational staff did not use good judgment and interfere when a child was put on a potential risk to injury while he was in care.

The operation did not provide a copy of the incident report on the day it occur to the parents within a 48 hour period. The parent did not sign and acknowledge the incident report either.

The operation does not have a current annual fire inspection since 10/30/2019.

The operation has not update their children product certification since 2020.

The operation is not maintaining the loose-fill in the toddler playground. The playground surface can be seen and not being maintain at six inches required by licensing.
