I observed a child under two watching television upon arrival for unannounced monitoring inspection. THis was corrected when Inspector asked how old the child was. The caregiver stated one and turned the television off.

The operation has a volunteer caregiver who did not have the required annual trainings available for review.

There was one child present at the operation who record was not available for review.

Six children's file were reviewed and four of them did not have Health Statement from a health care provider available for review.

Five children's records evaluated did not have health statement information on file.

I did not observe the children attendance record available for review.

The current lunch menu for July 2025 was not available for review.

The fire drills were not documented monthly and last one documented was March 2025.

Two children did not have required napping equipment available for naptime.

Child in care had a cell phone and was using it to watch cartoons.

The operation is not consistently keeping sign in/out logs.

The operational policies were available for review but did not have the required information.

Children were not observed with electronics, but were not engaging in full activities designed for each child's needs.

During the inspection, children were observed running around, climbing the table and chairs, climbing the sink in the bathroom, hitting each other while the primary caregiver did not have visual awareness of the children.

The admissions forms used were outdated. The admission forms do not capture the required minimum standards changes of March 2023.

The provider stated the updated operational policies are not available for review.

I observed children with personal iPhones and an iPad during the inspection.Inspector explained to the caregiver that the children should have an activity that does not consistently involve electronics. Caregiver collected the devices.

Provider has not submitted form 2962 or lack of liability insurance through provider portal.

The operation does not have liability insurance and has not notified parents.

The providers training records were not avaliable for review during inspection.

The operation has a 2A fire extinguisher.

I observed a high chair being used without safety straps and it is designed to have the straps.Inspector informed caregiver the children can not use high chairs with out the safety straps if the high chair comes with the straps. Caregiver removed the child from the highchair.

There is no documentation that CPR/First aid was completed by Ms. [name redacted]

There is no documentation in the emergency prepardness plan that states how children will be evacuated under the age of 24 months.

The fire extinguisher was sitting on the top of a cabinet. It was not mounted as required.
