One infant was observed sleeping, laying in a crib that was not bare, a blanket was in the crib with the child in addition to a bib.

2 personnel records reviewed were lacking all required training hours, 1 missing 10 hours, the other missing 18.25.

1 of the 10 personnel records reviewed had records that were not transcribed to English.

2 of the 10 personnel records reviewed were missing affidavits, form 2912. 1 record of the 10 had the affidavit; however, it was not notarized.

3 out 10 childrens' records monitored were missing some of the following: health care statements, authorization for emergency medical attention, and emergency contact address and phone numbers.

2 out of 10 child records were observed to be missing up to date immunization records. 2 additional records had immunization records that were not current.

In the 4 to 5 year old classroom, 8 electrical outlets were observed to not have safety outlet plug covers. ***this was corrected at inspection

In the infant classroom, one infant was observed asleep in a crib with a loose fitted sheet.

A caregiver used an inappropriate method of discipline when she grabbed a child by his forearm to redirect his behavior.

Seven children observed seated at the table with bucket type seats were observed to not be strapped properly in their seats.

The emergency preparedness plan submitted for compliance did not have an address listed of where children will be evacuated or how children will continue to care for children until parents arrive. This is a re-cite.

The emergency preparedness is not provided to parents in the operational policy. The emergency preparedness information that was on file did not have information on how children with limited mobility or who are under 24 months will be evacuated, how children will continue to be cared for until parents arrive, and an address as to where they will go if they need to evacuate.

The director was missing 17 out of thirty training hours. Some of the hours include but not limited to: Communicable diseases including immunizations, administering medication, building and premises safety, hazardous materials, and transportation training. One out of four staff was missing 24 hours of pre-service training. One out of four staff was missing 16 hours of annual training. The trainings include but are not limited to child maltreatment, sids, shaken baby, and early brain development, communicable diseases, and food allergies.

Inspector observed one staff in a classroom talking on a cell phone while children were napping. Inspector observed a second caregiver reading a book while caring for children who were napping and a third caregiver eating lunch while in a classroom with children who were napping. A fourth caregiver was observed coloring in a color book while supervising children who were napping.

Inspector observed one 9 month old infant asleep with a swaddle outfit on top of him being used as a blanket. Inspector asked that staff remove it from the crib and staff member put it in the corner of the crib.

Inspector observed a 2 month old infant swaddled while asleep in a crib. Inspector asked staff to take the swaddle off the child but the staff left the swaddle on and only took the child's arms out.

The standard was found deficient as part of a DFPS investigation.

Based on the information gathered by the DFPS investigation, a caregiver grabbed and threw a child on a table, causing bruising to the left and right arms, left shoulder and left lateral torso.

Staff used lollipops to bribe a child who was attempting to climb the fence and later left the operation unsupervised.

One staff did not communicate with other staff about which incoming children they would be in charge of.

Based on the information obtained during the course of the investigation, one child was not properly supervised and as a result left the operation and went into a high-traffic road. At the time of the incident, four staff were observed in close proximity to one another instead of caring for children.

At the time of inspection on 4/9/24 the operation was over their infant capacity. The operation's capacity for infants is 23 and they had 24 infants in care.

At the time of inspection the operation was using one room as an infant room without the approval of an amendment. The room has been used since March 2024 for infants however an amendment was not submitted until April 2nd, 2024 and has not yet been approved.

Inspector observed one infant 5 months of age asleep on stomach, when inspector spoke to staff they stated they placed the baby on her stomach to sleep.

The operation failed to submit and receive the results of a background check and FBI Fingerprint for one person who was present and had unsupervised access to children.

The operational policy on file was missing the following information: 1. Information on TB testing for children and adults 2. Transportation 3. Maintaining unassigned eppi-pens 4. Promotion of indoor/outdoor activity 5. Procedures for parents to contact the director for any concerns, or questions

There are no fire drills listed as of January 2024. The operation also has no documentation of checking the carbon monoxide, fire extinguisher, or smoke alarm.

Inspector observed two staff in two different classroom on cell phones. This citation was previously cited for an investigation. This is a re-cite.

The emergency preparedness did not have information on how children who are under 24 months or who have limited mobility will be evacuated or for how staff will continue to care for children until parents arrive.

Inspector observed two infants asleep in restrictive devices. One infant was four months in age and one was six months in age. One child was in a swing and the other was in a bouncer. Inspector observed the classroom a second time later during the inspection and an infant was again observed asleep in a swing.

Four out of four staff did not have complete training records on file. One out of four staff was missing training on Sids, Shaken Baby, and Early Brain Development, Food Allergies, and Emergency Preparedness. One out of four staff was missing six hours of training including child maltreatment. The caregiver needs in person training on file. One out of four staff had no trainings on file and requires pre-service. One out of four staff was missing training in child maltreatment, sids, shaken baby and early brain development, and food allergy.

Based on the information provided during the course of the investigation, staff were texting on cell phones while caring for children.

Based on the information obtained during the course of the investigation, staff at the operation failed to report suspected abuse/neglect within the operation.

Operation did not have a current fire inspection at time of follow up inspection. This deficiency will be recited.

The outside playground area did not have the required six inches of loose fill around the play structure which also has a slide.

Operation did not have a current fire inspection; the last fire inspection was completed on 4/8/2022.

At the time of inspection, a caregiver who had unsupervised access to children in care, was present with an ineligible background check.

One out of the two transportation vehicles did not have an electronic child safety alarm installed in the vehicle.

Two infant classrooms observed during lunchtime were not utilizing the safety straps on high chairs, half-moon bucket seats, and low-to-the-floor high chairs.

There is peeling paint and rust on a stationary piece of equipment used by children for climbing and sitting. The toddler play area has a gate that does not oepn freely as the bottom of the gate scrapes the ground.

The outside playground area did not have the required six inches of loose fill around the play structure which also has a slide.

One child's food allergy emergency plan was not signed by the parent.
