The caregivers on the playground were observed via video footage not properly supervising the children playing on the playground.

There was no current gas leak inspection. The last gas leak inspection was conducted on 1/16/23.

Based on the information provided during the course of the investigation, two children were observed in infant classrooms who were more than 18 months older than the infants in the classroom. The children were approximately 11 and 13 yrs. old.

The director used poor judgment in allowing an 11 and 13 yr. old to go into children's classrooms and carry infants.

The operation allowed two children to be present at the facility who were not enrolled. The governing body admitted that the children were not enrolled and no paperwork was provided to Inspector at the time of the investigation inspection.

Based on the information provided during the course of the investigation, the director failed to operate in compliance with minimum standards.
