A caregiver mopped a floor that was accessible to children in care afterwards.

It was found that a caregiver did not use good judgment when the caregiver improperly lifted/pulled a child's arm.

An incident report for an incident where a child was alone unsupervised for approximately 15 minutes was not completed and shared with the parent(s) and signed in the required time frame of the incident occurring as the incident occurred on 8/14/25 and the report was signed by the operation and parent on 8/18/25.

Information gathered in the investigation substantiated that a child was alone unsupervised for approximately 15 minutes when they were able to get into the classroom from outside and then exit the classroom into the hallway and enter the laundry room which was not locked before being found by a staff member.

A caregiver had only completed 9 hours of pre-service training and was short 15 hours of 24 hours at their 90-days of employment. Their hire date was 5/12/25 and the 24 hours of pre-service was not completed by 8/10/25.

Information gathered in the investigation substantiated that a parent(s) was not notified immediately when their child was found alone unsupervised inside the operation. The incident occurred on 8/14/25 in the morning and the parent(s) was not notified until 8/15/25 after 9 pm.

One caregiver had first aid/cpr that expired 1/16/25.

The last annual fire inspection was conducted 1/10/24 which is over 12 months ago.

During the investigation inspection in the younger infant classroom, a caregiver told me there were 7 children present, but there were 9 children present. Information gathered in the investigation substantiated two caregivers of a child left alone unsupervised did not know which children they were responsible for as the counts were off and it was not noticed the child was missing and not on the playground when the class was playing outside.

An incident report was not completed as required for a non-routine situation that occurred in care that placed a child at risk.

Supervision was not maintained as required when a child in care was left alone unsupervised for approximately twenty-three minutes during a transition from the classroom to the bathroom and playground.

A caregiver involved in the incident being investigated was missing 13 hours of the required 24 hours of annual training.

The last documented health inspection was conducted 9/10/23.

The operational policies did not include the criteria used to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play.

There was medication for a child that expired 2/24. There was also an expired burn spray with the first aid supplies. Note: This was corrected during the inspection when the expired medication and burn spray was removed and disposed of.

The mulch under the play structure, under the swings, and on the small structures between those two sections of the playground was not at the proper required depth which was evident even without measuring. Under one of the slides on the larger play structure, the black tarp underneath was coming up. Note: Photos were taken.

Information gathered in the investigation substantiated that a caregiver did not use good judgement by not asking for assistance when they were struggling with a child who did not want to lay down and sleep at nap time. Information gathered supported the caregiver knew they should have called for assistance but did not nor did they ask their co-teacher for help.

Information gathered in the investigation substantiated that a caregiver grabbed and pulled a child when stopping them from running and while trying to get them to go to their nap mat and to lay down.

Information gathered in the investigation substantiated that a caregiver inappropriately tried to get and keep a child to stay on their nap mat and sleep by wrapping them in their blanket as well as trying to keep them in place on the mat.

One caregiver had first aid/cpr that was expired.

During nap time in the older infant classroom that had a specified age group of 12-17 months old, there were 12 infants with one caregiver.

The annual liability insurance verification was not completed as required. Note: The verification was completed during the inspection.

1 of 10 children's records evaluated had an immunization record that was not current. Two required immunizations were missing for the child.

1 of 5 personnel files evaluated for training was short 12 hours of the required annual training.

An infant under 6 months old was not being held while having a bottle.

A monthly fire drill was not conducted in June 2023.

A child with a food allergy plan that requires an epi pen had an epi pen that was expired.

The feeding instructions for all of the infants in the younger infant room had not been reviewed and updated by the parent in the last 30 days.

The operational policies do not include a policy on the promotion of indoor and outdoor physical activity.
