The back outdoor playground area contained hazards such as 2 feet of rebar sticking out of the ground near the swings. The sidewalk is a tripping hazard due to uneven, broken concrete. There are various broken toys, furniture and trash on the back outdoor playground. The surfacing is not maintained under the swings. Lastly in the front play area accessed by the children there is a hole in the side of the building where a stray cat gained access. The children will not have access to the back play area until compliance is maintained.

The smoke alarm in the afterschool classroom was not working due to the battery being installed incorrectly. This was fixed at inspection. The smoke alarm in the infant room had been moved and had not been reinstalled into the infant room. This was corrected at inspection.
