During inspection of the playground there were seveal empty plastic water bottles and a broken plastic shovel. The violation was corected at the time of inspection. The bottles and damage shovel was removed from the playground.

A caregiver stated that they did not hold a preemie infant with an adjusted age of 6 months old for their feeding.

An infant was left unsupervised for 1-2 minutes swaddled on the floor with a propped bottle.

The operation did not have infant cribs labeled.

The operation did not self report a incident where a parent observed their special needs infant laying on a floor unsupervised with a propped bottle.

A caregiver who has received supervision training did not demonstrate competency when they left a special needs infant unsupervised for 1-2 minutes.

The operation did not provide a infant daily report to share with the parents on 4/27/22.

The operation did not have a current fire inspection.

Electric outlets were not covered in the infant class, two year old class, the hallway, the restroom and a common area classroom.

Three medications did not have authorize forms signed and dated by the parents authorizing the operation to administering medication.

The operation did not complete a fire drill since August 2021. This was corrected at inspection when the operation conducted a fire drill during the inspection.

Out of four staff members records reviewed, three staff members did not have certificates available for review at the time of inspection.

During a fire drill a caregiver left one child in the classroom unattended. The director was able to retrieve the child and bring the child to its group. This was corrected at inspection, when the director came up with a plan to ensure name to face checks were conducted during all transitions. One child was observed going to the restroom by itself. There were no caregivers near the restroom or standing in the hall to supervise this child. The director was informed and a caregiver was sent to supervise the child in the restroom.

The operation did not have a current fire inspection.

One Epi-Pen was not in its original container.

The operation did not have documentation available at the time of inspection showing smoke detectors and carbon monoxide detectors were tested. This was corrected at inspection when the director tested all detectors and documented on the form.

The operation was in need of a deep clean as old dried urine was visible in the boys restroom, dead inspects were visible on floors, dirt was visible on walls, and dust particles were visible on toy shelves used by children.

Out of six files reviewed, two children did not have immunizations available for review at the time of inspection.

The operation did not have a monthly maintenance checklist completed. This was corrected at inspection when the director completed the maintenance check during the inspection.

Out of four staff files reviewed, two caregivers did not have current certification of CPR and First Aid training available for review at the time of inspection.
