A caregiver, who previously worked at the center, was present in the classroom without an active background check. Note: This was corrected at inspection when the caregiver was removed from the classroom and her background check was resubmitted.

One out of the ten staff files reviewed did not have the total required 24 hours of required trainings available from the last full employment year.

Based on a DFPS investigation, video footage showed a caregiver left a group of children unsupervised for less than one minute when she exited the classroom.

One of three transportation vehicles did not have a safety alarm installed. The 2018 vehicle seats 12 passengers. This was corrected at inspection when the vehicle was pulled from service. The vehicle must be re-inspected prior to being put back into service.

During snack, the children were served veggie sticks, salad dressing and water, which only meets one of the three required components that must be included per the USDA Nutritional Guidelines.

During a DFPS investigation, it was determined that caregivers did not notify a parent of a minor incident/injury that occurred to their child while in care that day.

At the time of inspection one child sitting in the high table was not strapped in the chair. This was corrected when the strap was fastened around the child.

At the time of the inspection one caregiver left the second caregiver walked to the kitchen leaving the caregiver with eight children ages 12 to 17 months of age. This was corrected when the caregiver returned to the classroom with the caregiver and the group of children.

At the time of inspection one child's medication was not in the original container.
