Four out of five child records evaluated were lacking an admission date. This was corrected at inspection when the admission dates were added to all records.

A caregiver without an active or eligible background check was present in a classroom at the operation. This was corrected when the caregiver was asked to leave the operation.

The director was lacking 16 out of the 30 annual training hours for the most recent year completed.

Two out of five child records evaluated were lacking immunization records.

Two out of three employee records evaluated had out of date CPR training.

A caregiver was lacking 11 out of the 24 annual training hours for the most recent year completed.

The center was missing the inclusion services with children with special care needs.

The center did not have proper amount of loose fill for slide that was approximetly 6 feet tall.

The operation did not have a use zone of 6 feet for the ladder that leads to 6 foot slide.

The center was missing criteria for extreme weather in their operational policy.

The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.

Cribs was not labeled to identify which child sleeps in each crib. CI: This was corrected hen the caregiver labeled each crib with the child's name.

The operation lacked a current fire inspection.

There were 36 children observed to be present in care, however the operation's capacity is 35.

Two out of four children's records lacked immunization records.

An eleven month old infant was observed sleeping in a crib while drinking a bottle.

One out of three staff files reviewed did not have training in the following areas for review. Pre service training with SIDS.

One caregiver was observed with five infants with the specified age of 11 months old.

A caregiver was observed to be in charge of 5 children, who had not received eligibility notice to work at the operation.

The Director had six of the required thirty hours of training for the last full year of employment.
