In the 18 month to 22 month old classroom, the children were watching the wheels on the bus video on an ipad. The caregiver corrected the deficiencies, by turning off the ipad and reading to the children.

There were 2 out of 5 classrooms, that told child care regulation the wrong ages of the children.

During the investigation, there was evidence to indicate a caregiver pushed a child with force.

In the infant room. there was a tablet out playing a music video. This was corrected at the inspection by removing the tablet from the children's view.

The operation did not write an incident report until after the parent picked up. The operation did email the parent the incident report after the incident and the child was dis-enrolled

Upon arrival two children under two years of age were watching television. The Director was informed of the deficiency and Operation Staff turned off the TV and imediately moved to an alternative activity.

The air vents in the building had a buildup of lint and other allergens. There were cobwebs in various locations in the building. Live and deceased pests were observed in classrooms and the walls of the common area.

During a walk through a classroom with children under the age of two present was seen having screentime. This was corrected when the t.v. was taken out of the room.

While reviewing the operational policy there was no safe sleep guidelines available.

While reviewing required postings it was seen the annual certification was last signed 1/3/2022.

While reviewing the operational policy there was no promotion of indoor and outdoor play available for review.

During a walk through TV cords were exposed and accessible to children.

During a walk through there were no hand washing sinks available in two infant rooms.

During a walk through there was no outdoor space available.

While reviewing 4 staff files all CPR training was completed online.

While reviewing the first aid kit there were no tweezers available. This was fixed when the tweezers were added to the first aid kit.

While reviewing staff files 1 out of 4 files did not have a photo identification available.

While reviewing the required posting there was no current employee list available for viewing.

While reviewing records 3 out of 4 staff did not have proof of DFPS background checks available.

During an assessment it was found the operation changed locations without notifying CCR.

Observed one infant sleeping in a car seat and observed another infant sleeping in a bouncy seat. This was corrected when both children were removed from the restrictive devices and placed in cribs.

The operation does not have written feeding instructions for any infants in the operation.

Observed an infant sleeping with a bottle. This was corrected when the bottle was removed from the crib.

Observed 15 infants in a classroom with 3 caregivers. The maximum group size for infants is 10. This was corrected when 5 infants were moved into the other infant room.

Observed two infants sitting in rocker chairs. The infants were in the chairs at 11:30 when the inspection started and while going back to the infant room at 1:00, the same two infants were still sitting in the rockers. This was corrected when the infants were removed.
