Operation staff did not demonstrate good judgment due to not notifying CCR regarding the relocation of the infant classroom and have CCR inspect the new room to ensure it met infant care requirements.

While reviewing records it was found carbon monoxide dectors were not being tested monthly. This was fixed when the carbon monoxide detector was tested during the inspection.

While reviewing records it was found smoke detectors were not being tested monthly. This was fixed when the smoke detector was tested during the inspection.

A first aid kit had solution present with an expiration date of 2016. This was fixed when the solution was removed.

During a walk through an infant was observed sleeping in a restrictive device.

While reviewing records there were no health statements available.

While reviewing emergency practices it was found drills were not being conducted. This was fixed when the operation completed a fire drill during the inspection.

During a walk through an infant was sleeping in a restrictive device. There was another infant present in a crib that was not bare. This was fixed when the infant was removed from the restricted device and crib was made bare.

It was found the operation did not report a incident in a timely manner.

While watching a video a child was seen standing in timeout for approximately 8 minutes. After caregiver interviews it was found a caregiver displays prohibited punishment by yelling at children in care.

During caregiver interviews it was found that a child was left in a room unsupervised.

There were unmounted fans in 4 classrooms. This was corrected at inspection when the fans were removed from the classrooms.

There were no background checks for a person listed as the Governing Body-Designee. *This was corrected at inspection when the initial background check was submitted.

The background checks for one subject has been pending eligibility since 4/7/2022, which is 49 days past the initial submission. The last validation of the Background Check list was on 5/10/2022. *This was corrected at inspection when the subject was removed from the list.

During the investigation it was found that the operation did not use good judgement when they did not communicate to the parents that the afterschool program was closed and that transportation would not be provided that day.
