During the unannounced inspection, there was not a working smoke detector in the playroom. A new smoke detector was installed in the playroom during the inspection.

Fire drills are not being conducted monthly. Also checking smoke detectors, carbon monoxide detectors, and fire extinguishers are not being done monthly. A fire drill was conducted during the inspection allong with checking the fire extiguisher, smoke detector, and carbon monoxide detectors.

Provider did not have proof of current first aid and CPR.

The provider did not have proof of vaccinations for two of their three dogs.

There was not a working smoke detector in each room used by day care children.

The permit holder was over ratio. The permit holder was caring for a 9 month old, 9 month old, 2 year old, 2 year old, 3 year old, 3 year old, and four year old. This was corrected during the inspection placing the operation back into compliance.

The provider did not have a sign in sheet available for review.

The child safety lock under the kitchen sink was not functional. The children in care had access in the kitchen area to chemicals located underneath the sink. The gate to the backyard was not attached to the fence. There was an opening to the backyard where the gate was supposed to be placing the children at risk as the backyard was not enclosed. The fence was not flush to the ground and exceeded 3.5 inches presenting an entrapment hazard where the children in care could crawl underneath to leave the backyard.

The weapons located in the glass gun safe were not locked. The gun safe was open and accessible to children in care inside the residence. The gun safe was immediately locked during the inspection placing the operation back into compliance.

The operation was missing the following policies and procedures: handling medical emergencies, discipline and guidance, procedure for parent to review a copy of the minimum standards and how to access the most current inspection report, instructions for contacting the local Child Care Regulation office, Abuse/Neglect/Exploitation hotline, and HHS website. The provider was also missing promotion of indoor and outdoor activities with the duration, recommended clothing and footwear, and plan for physical activity during inclement weather conditions.

The ammunition for the weapons was sitting on top of the glass gun safe. The ammunition was not locked in a separate area from the weapons.

Two infants in care not yet on table food did not have any past or current infant feeding instructions to review.

Two 9-month-old infants were observed with bottles in the playyards while they were sleeping. The permit holder verified that the bottles are utilized to aid the infants while going to sleep. The bottles were immediately removed during the inspection placing the operation back into compliance.

One of the four children records audited did not have a health statement available for review.

The assistant caregiver did not have proof of the background check request or eligibility letter. The assistant caregiver did not have a photo ID or driver's license in the personnel file.

The permit holder was over ratio. The permit holder was caring for a 9 month old, 9 month old, 2 year old, 2 year old, 3 year old, 3 year old, 4 year old and 4 year old.

Two 9-month-old infants in care were observed sleeping in a playyard with blankets, stuffed animal, and bottles. The bottles, blankets, and stuffed animal were immediately removed, placing the operation back into compliance.

The last inspection report was not posted with the required postings. The list of employees was not posted with the required postings.

The carbon monoxide detector located in the kitchen area did not work when tested during the inspection.

The assistant caregiver did not have a high school diploma/GED available for review. The assistant caregiver did not have a notarized affidavit of employment available for review.

There were 6 outlet covers observed to be missing, and there was a power strip laying on the floor in the living room with exposed outlets.

The child product safety form was outdated.

The operation did not have a written activity plan to review.
