The gate to the backyard was not attached to the fence. There was an opening to the backyard where the gate was supposed to be placing the children at risk as the backyard was not enclosed. The fence was not flush to the ground and exceeded 3.5 inches presenting an entrapment hazard where the children in care could crawl underneath to leave the backyard.
