During the inspection an infant who could not roll over independently was observed in the crib face down.

Sole caregiver in two year old classroom reported having 11 children. Caregiver was present with 13 children.

Sole caregiver was observed with eight infants with a specified age of 17 months. Corrected when a second caregiver was added to the classroom.

While doing the walk through at the operation I observed two infants sleeping on sheets that were not tight fitting.

While doing my walk through I noticed in 4 different rooms there was outlet plugs that were not covered. In all 4 rooms the kids were under 4 years of age.

While doing the walk through we went into an infant room there was 1 caregiver with 5 infants the children were 8m, 9m, 9m 10m and 12 month. The allowed ratio for that age group is 4 children to 1 caregiver.

While doing my walk through I observed a baby sleeping in the swing. I asked the caregivers how long she had been asleep they said 30 minuets.
