Benbrook Elementary - Westside YMCA

There were thirty children present aged four to eleven years old with one caregiver. This was corrected at inspection when a second caregiver showed up.

During the inspection a seventeen year old caregiver was observed alone with a group of children while the other staff took children to the restroom.

During inspection a parent brought in a dog and the children were observed petting it then failed to wash hands after.

During the inspection I observed children walk into a electrical closet unsupervised, behind the curtain on the stage unsupervised, and a child crying in the hallway alone by exit door.

The operation was currently caring for Pre-K age children which they are not licensed for at time of inspection.

The operation failed to report to CCR they would be adding Pre-K age children to the program.

During the inspection I observed a seventeen year old caregiver take children to the restroom alone without another caregiver present.

During the inspection the site director was under 21 years old with Pre-K age children.

During the inspection the operation had not conducted a Fire Drill for February.

The program director and site director did not have copies of their director's certififate.

The programs display board was not set up at the start of the inspection. The violation was corrected during the inspection. The site-director set the board up.

The program did not conduct a fire drill in March 2023. The violation was corrected durng the inspection. The site director had to children to conduct a fire drill during the inspection.

This standard was evaluated as part of an inspection and found to be deficient when the children in care did not have an individual storage system in place for belongings.

For approximatley 15 minutes the program was out of ratio. There were 45 children with two caregives.

The program failed to report to Child Care Regulation and incident that occurred on 11/1/2021. A child fell from playground equipment and received an arm injury which required medical treatment.
