A review of the Employees Who Left Employment in 2025 data from January 20, 2026, determined the center had not reported the number of employees who left employment in 2025 by January 15, 2026, as required.

During an investigation, there was evidence to support that a caregiver at the operation was under the influence of alcohol while caring for children.

During an investigation, it was found that operation staff was aware of a caregiver being under the influence of alcohol while at the operation and did not report this to Child Care Regulation.

There were activity plans for February posted in 3 out of 4 classrooms. There was only one classroom that had an updated March Activity plan. The March lesson plan were added to each classroom.

Observed a 10 year old child in the infant room, and there was more than a 10 month age gap between the youngest infant at 8 months old to the 10 year old.

The operational policies were missing the following; Transportion, water activities, field trips, Criteria used to determine extreme weather, insect repellant/sunscreen, parent communication, parent participation, how parents can review minimum standards, parent participation, nursing mothers, health checks, employee vaccinations, preceedures for inclusive services for children with discibiliites, gang free zone.

Observed 5 children in the infant room ages 8m-10 years old. This was corrected when the 10 year old was taken to another classroom. The specified age for this classroom was 0-11 months.

Observed activity plans being written down on windows, and white erase boards. The activity plans were missing the dates, outdoor activities for infants and specific details of activities that will be completed each week/month.

Observed a 7 month old infant with children who are 4 years old. This was corrected when a caregiver came and they seperated the classes.

Observed one child sitting in a classroom by themselves. There was no caregiver in the classroom. I spoke to a caregiver asking them where the child belonged, and the caregiver took the child back to their classroom. The caregiver was unaware that the child was missing from the classroom.

One caregiver stated that they had 8 children in the classroom. As the kids were counted by the licensing Rep, there were 9 children in care. The caregiver stated that one child had just arrived. There was a child in another classroom alone that also belonged to this classroom which would have made the caregiver have 10 kids total.

After a review of employee files, there was no annual training regarding SIDS, Shaken Baby, nor Early Brain Development for director.

There was no annual training received regarding the following requred topics: administering medication, preventing and responding to a food or an allergic reaction, nor handling, disposing, and storing of hazardous materials.

Two caregivers did not have training regarding SIDS, Shaken Baby, and Early Brain Development. The caregivers were providing care for children younger than 24 months old.

It was determined a pair of scissors was left on a shelf accessible to children in care.

It was determined during the incident the caregiver was not properly supervising the group of children when the incident occured. The caregiver was preforming other duties that did not allow proper supervision.
