Director's transportation trainings was observed expired during the inspection time.

Five employees did not have completed annual trainings in their files.

A child was missing inmunizations records in the file.

Four staff have expired CPR/First aid trainings certificates.

Staff caring children with special care needs did not have specific topic of training in their files.

Three employees were missing pre-service trainings during the inspection time,

A child was missing a health statement in the file.

It was observed that the children's bottles in five classrooms were not labeled.

The infant's feeding instructions were not updated. Documentation shows the forms were updated a month ago.

During the inspection an eleven years old child was observed riding in the front seat of the van.

There was no observed surfacing material in use zones around and under the rotating or moving equipment.

Infant feeding plans for 9 infants were not current.

Three seperate classroom's caregivers did not know the number of children in each classroom. School Age said there were 16, but 15 were present. Four year olds said there were 16, but 17 were present. Two year olds said there were 10, but 11 were present.

Based on investigation findings a child was left at the location of a field trip. This child's name was not written on the list because the child arrived after the field trip began.

A caregiver present in the two year old classroom did not have an eligible background check. Note: The caregiver left during inspection and the background check was submitted.

An incident report was not provided to the parent of the child who was inappropriately disciplined on April 29th within 48 hours.

Based on findings during an investigation a three-year-old was inappropriately disciplined by spanking and kicking the child.

A 3-year-old child was disciplined inappropriately. The operation notified parents 4 days after the incident occurred.

Based on information obtained in the investigation there is sufficient evidence to show that a Director did not ensure compliance with the minimum standards when they engaged in prohibited punishment and did not report the incident to Child Care Regulation.

Booster seats on transportation van were expired. Note: Seats were removed from van during inspection.

The transportation van does not have a child safety alarm installed on it.
