At the time of inspection a child was found in a room alone and unsupervised.

A caregiver was witnessed on video grabbing and infant in the crib by one arm and turning them.

At the time of inspection the caregivers were not following a written plan and the classroom was not in control.

At the time of inspection the school age room was not in control, children were running around, horseplaying and did not have any planned activity they were engaged in.

A caregiver was observed on video grabbing an infant by the arm and leg while in the crib.

A Caregiver failed to demonstrate self control when she was witnessed on video, frustrated with infants, and roughly shaking an infant, and roughly using her foot to shake an infant in a bouncer.

An infant caregiver at the time of inspection did not have current SIDs, Shaken Baby training.

This standard was evaluated in regards to a report and found deficient. The school age room is not being properly maintained ,this is evident by overflowing trash as well as unclean bathrooms.

Floors and walls in the school age area were dirty and not propelry maintained at the time of inspection.

This standard was evaluated in regards to a report and found deficient. A caregiver grabbed a child by the jacket to correct him when he continued to use foul language in the classroom.

This standard was evaluated in regards to a report and found deficient. It was determined caregivers were using candy as a reward in the classroom.

During my inspection I witnessed a caregiver harshly grabbing a child's face when she was disciplining him on the playground. The director contacted corporate and the caregiver was sent home for the day.

A child ran out of the classroom and was able to leave the building unsupervised.

The caregiver in the infant room did not know how many children were in her care at the time of inspection.

The operation failed to report to licensing when a parent expressed concerns her child was being mistreated by a caregiver.

One of the 4 vans evaluated during the inspection did not have a First Aid Kit. The director corrected during the inspection.
