Information obtained throughout the investigation shows that a child was not thoroughly cleaned during a diaper change, resulting in a diaper rash.

Two children were missing parent signatures on their food allergy plans. One child was missing a doctor's signature on their food allergy plan.

One child with a diagnosed food allergy did not have all of the medication listed on their food allergy plan present at the operation.

Three children with diagnosed food allergies had expired medication.

The operation did not document the dates that the fire extinguishers, smoke detectors, and carbon monoxide detectors were checked. This was corrected when the director completed the documentation.

Three caregivers were missing annual training hours. One caregiver was missing 4 hours, one caregiver was missing 24 hours, and the last caregiver was missing 7 hours.

The operation did not have a current fire inspection. The most recent expired inspection 7/16/25.

Two epipens were not in their original containers.

A caregiver, with direct access to children, was present without a background check eligibility. The caregiver was sent home during the inspection.

Two children did not have health statements on file.

One caregiver did not have current in person CPR training.

Operation has not updated liability through the provider portal as discussed on 5/7/25 during the inspection.

Three caregivers did not have affidavits on file.

One caregiver did not have current first aid training.

One caregiver did not have proof of education on file.

Two caregivers did not have orientation training.

Operation did not have documentation that smoke detectors are being tested monthly.

Two caregivers did not have proof of preservice training.

One caregiver hired after 9/2023 did not have a pre-employment affidavit on file.

Three children did not have a health statement on file.

Two children did not have the address of their emergency contacts listed.

The operation has did not have a current health inspection on file.

There was expired benadryl on the premises.

Three caregivers did not have first aid training.

A caregiver was observed diapering a child and allowing the child to go back and play without washing their hands.

A caregiver walked out of her classroom and left the children unattended briefly during my inspection.

The director did not have the required 30 hours of annual training.

Five caregivers did not have the required 24 hours of annual training.

Three caregivers did not have hands on CPR training.

The last compliant fire inspection was observed to be completed in 2022. A fire inspection completed on 4/3/24 required corrections that are not yet completed.

Three of four personnel files reviewed were found to only have online CPR training certificates available for review. The in person aspect of the training was missing.

Three of 8 childrens' files reviewed were lacking a physician signed health statement.

The first aid kit did not have gloves or the guide to first aid and emergency care. The director was able to add both items to the kit to make it complete at inspection.
