An infant was laying on top of a soft blanket in the crib.

This standard was found deficient as part of a DFPS investigation.

It was found during the course of the investigation that the caregiver pushed a child off a chair when they did not follow instructions.

A child received an injury after being hit with a shoe by a caregiver.

One infant in care has not had updated feeding instructions since January 2026.

A caregiver placed an infant in a bouncer and did not buckle the straps. This was fixed at inspection when the inspector requested the caregiver buckle the infant in.

The bathroom floor was lifting and needs repairing, and the floors need to be cleaned around the toilet.

At the time of the inspection, 2 infant bottles were not labeled with the names of the children using them.

An infant was asleep in a crib with a pacifier clip string which presents a strangulation hazard.

A classroom where diapers are changed did not have a sink to wash hands in after.

The first aid kit was missing adhesive bandages, displosable gloves, scissors, and the antiseptic wipes were expired.

Peeling paint was observed on the wall in the children's bathroom and water damage was observed on the ceiling in a closet.

Parental notification was not provided when a child in care was injured.

At the time of the inspection, there was an infant in a baby walker in the infant classroom. It was removed during the inspection.

The annual fire inspection is expired at the time of the inspection.

At the time of the inspection, the infant classroom had 6 infants and 1 caregiver.

THe health inspection was expired at the time of the inspection.

The caregiver had eleven children with one infant and was out of ratio. This was corrected at inspection when the director arrived at the center five minutes later.

In one of the room the electrical outlet was not child proofed. This was corrected at inspection when the director Ms. [name redacted] child proofed the outlet by placing a child proof cover on it.

The caregiver had 14 children with a specificied age group of 2 years upon inspector's arrival. Another caregiver arrived approximately 5 minutes after this inspector's arrival.

A cushion was being used with a cloth sheet covering in the diaper changing table. *This was corrected during the inspection when the cushion was removed, and the surface was not absorbent.

Toxic Chemicals were being stored near the food served to children.

There was peeling paint on the wall, and peeling sheetrock on the ceiling on one classroom. The wall in one classroom had dry rot that was covered with wallpaper. and tape. There was dust buildup on the vents cover. The sinks in the bathroom had low water pressure. Some of the play equipment was missing pieces such as: 3 tricycles were missing foot pedals. There were spider webs, and bird droppings in the play equipment used by children in care. Dirty build up on food preparation equipment and in the refrigerator.
