The last santation inspection is dated 09/05/2024.

There are concerns with caregiver to child ratios.

The caregivers in charge of the classroom were not aware of the number of total children in their care.

It was found that during the break transition of teachers it was not communicated that a food item should not be served to a child with a food allergy.

Classroom preschool 1 caregivers reported having 17 children in care when they had 20 children.

A child was left unsupervised for 5 minutes in a classroom.

There was a child that was left unsupervised in a classroom.

The operation failed to report an incident where a child was left unsupervised in a classroom.

At inspection, the operation did not have a playground checklist, available for CCR.

A child was left in the classroom alone for approximately 4-6 minutes.

Of the 10 staff files reviewed the following needs to be updated: 4 files need the date of hire 5 files need documentation of Pre-Service Training 4 files need to complete 20-21 training hours 7 files need to complete 21-22 training hours.

Updated infant feeding instructions were missing for 8 children. Two of these were corrected during inspection when the parents were contacted and stated that there were no changes.

One staff member was accidentally inactivated on 2/23/22. They were present at the operation without an updated background check eligibility notification. This was corrected at inspection with the staff member left the operation while I was present.

10 staff files were reviewed. One staff member needs proof of her 2020-2021 trainings, and one staff member needs proof of two more hours of training.

One allergy action plan needs to be signed by both the parent and the doctor. Two allergy action plans need to be signed by the doctor.

One child was missing an up to date immunization record.

Expired medication was located in the medication bin during the inspection. This was corrected at inspection when the expired medications were removed from the medication bin.

5 staff training files were not available for review during the inspection.

During naptime one classroom was out of ratio. There were 10 children in care with one staff member. There were children in the room under 18 months old. This was corrected at inspection when the second caregiver returned to the classroom.

Two staff members were unaware of the number of children in their classroom.

Two staff members need notarized affidavits for the HR file.

Two classrooms had exit doors that were blocked by naptime cots. This was corrected at inspection when the cots were removed from in front of the doors.

A copy of the updated annual fire inspection was not on file at the operation.

Drill logs have not been documented since August 2021.

One caregiver needed to renew their CPR/First Aid training.

Three caregivers were missing the notarized affidavit in their staff file.

10 staff files were reviewed. Six caregivers were missing training in child maltreatment.

One caregiver was missing a copy of their diploma or transcripts in their staff file.
