At inspection, 2 caregivers did not have current active background checks. The 2 caregivers left during the inspection.

At inspection, video footage showed that a child that was awake, was not removed, from their crib for a period of one hour and 15 minutes.

There was a child whose EpiPen expired in April 2024.

The operation lacked documentation showing that monthly checks of smoke detectors, fire extinguishers, and carbon monoxide detectors were completed.

A child had an EpiPen that was not in its original container and was only labeled with the child's name.

Two infant classrooms were out of ratio during naptime.

The provider was not able to find any recent playground maintenance checklists.

The front playground had a play structure with slides that did not have loose fill surfacing underneath it.

The operation allowed a person to be present at the operation prior to receiving a background check determination of Eligible , Provisionally Eligible With Conditions , or Eligible With Conditions .

The operation did not practice a fire drill one month (June 2022) out of eight months. This was corrected at inspection, when the director conducted a fire drill.

There were five infant chidlren (0 - 11 months) to one caregiver, placing the caregiver out of the child/caregiver ratio.

There is sufficient evidence to support that a caregiver has yelled at the children while in care.

The Infant B Class had 5 infants with an SAG of 0-12 months and only 1 caregiver was present. Co-Director was able to add another caregiver to the class during the inspection until the second caregiver assigned to the room returned from break.
