The operation failed to share a copy of the incident report with the child's parent and obtain a signature, within 48 hours of the incident.

There was dust build-up on the ceiling tiles and vents in multiple classrooms. There was a piece of the wall/sheetrock detached in one classroom

A child was left unsupervised 20 minutes in a classroom.

It was found that a child was left unattended in the hallway for about approximately one minute.

A child was left unattended in the classroom in the crib with no caregiver present.

There are concerns about caregivers speaking negatively towards children.

A 2 year-old child was left in a fenced playground for 3 to 5 minutes after the child's group transitioned inside. There were other groups of children in close proximity, but not on the same playground as the child.

Four children had expired medication present at the operation as part of an emergency allergy plan.

One child's allergy medication was not in the original packaging. It was stored in a ziploc bag without any label on it.

Two children did not have a completed emergency allergy plan as it was missing one of the two medications listed as required by a physician.

Three children did not have current immunization records.

One staff was present at the operation, and had access to children in care who did not have an eligible background check. Another staff submitted a background check during inspection, and the staff without an eligible background check was observed leaving the operation during inspection.

One caregiver did not know the ages of the children she was responsible for. This was corrected when she was able to locate the correct ages for the children during inspection.

One infant was left alone in a classroom.

Documentation for change in director was not received prior to the comply by date of the previous citation.

Director changes for the preschool program was not reported to CCR.

During the course of the investigation, it was found that a child was left unattended in a classroom during a transition to the playground.

A caregiver yelled at a child and pushed the child into a chair.

One expired Epi Pen was located during the follow up inspection.

The operation had two expired medications on site.

10 staff files were reviewed. One Staff Member needed food allergy training. Two Staff members needed training in hazardous materials, building safety, and emergency preparedness.

The operation did not have a current playground checklist available.

The operation was missing the signed annual recall product certification form. This was corrected during inspection when the recalls were reviewed, the certificate was printed, and it was posted on the parent board.

One allergy action plan was not signed by the parent.

One classroom was missing an evacuation plan. This was corrected at inspection when the Director printed out a new plan and posted it in the classroom.

Caregivers did not use good judgment when deciding to both change diapers at the same time, which allowed a child to be injured as neither caregiver could quickly intervene.

Based on the the information obtained during an investigation conducted by DFPS, it was found that a caregiver demonstrated poor judgment when forcefully placing a child in a crib resulting in an injury.
