A child was missing 3 of the required immunizations.

During an investigation it was found a caregiver used prohibited punishment.

Based on the evidence, it was determined that the caregiver had grabbed and pull multiple children in care. This was corrected when the caregiver was removed from the school.

It was determined that the caregiver was using harsh language toward the children in care. This was corrected when the caregiver was removed from the school.

There is enough evidence to determined that a caregiver was yelling at the children in care. This was corrected when the teacher was removed from the school.

One child's immunization records did not have the signature (including a rubber stamp or electronic signature) of the health-care professional who administered the vaccine.

None of the operation's fire drills that were conducted had an exit time of three minutes or less.

The operation did not have the following documentation: fire drills completed for five months, Fire Extinguishers were inspected for seven months, four lock down drills were completed in 2024, and three severe weather drills were completed in 2024.

The loose-fill under a slide did not have the required 6 inches of loose-fill. This was corrected at the inspection when loose-fill was raked into the area up to 6 inches.

Three out of ten children's files did not have an emergency contact address. This was corrected at the inspection when the director contacted parents and input the emergency contact adresses.

It was determined a child was left unsupervised in a classroom for approx. 2 mins.

The operation does not have a current sanitation inspection.

Information obtained throughout the investigation shows that parents were not immediately notified of a situation placing children at risk.
